- Print out the VCUHS Access Request form.
- Complete the form with the following information:
1. Contact name and information 2. Suggested site name. 3. Indicate if you need to post Bulletins or open Discussion Rooms. 4. Obtain supervisory signature.
- Once completed, fax your request to the Hospital Information Systems Department at 828-6132.
- You will be contacted when your site has been established.

Your site name will be placed in front of the VCUHS domain(.MCVH-VCU.EDU) to uniquely identify your site. For example:
